Here's the wording from the "Data Access Modification Procedure":

An existing employee requiring new or modified access to an Information Resource should initiate the request via their supervisor. The supervisor will then communicate with the respective Vice President who will involve any other applicable Vice Presidents, and finally to Information Services Staff to perform the approved change.
